Life and job changes can bring new opportunities and challenges. Whether you’re starting a new job, moving, or experiencing significant life events, our resources are designed to guide you through the impact on your benefits and coverage.

When and What to Update

It is your responsibility to advise UA Local 67 Benefits when there is a change in your union status, family status, beneficiary designation, or contact information, to prevent interruption to your coverage.

Follow the guidance below and contact UA Local 67 Benefits in these life events:

Life Changes

Name, Contact, or Banking Information Changes

Please inform UA Local 67 Benefits of any changes to your address, phone number, email, or name for you and your dependents. Keeping this information up to date is essential to ensure your claims are processed and reimbursed correctly, and you receive important updates from UA Local 67 Benefits and our partners.

Retired members receiving Pension Plan benefits should be sure to update their banking information with UA Local 67 Benefits to avoid any delay in receiving your pension.

Spouse Changes

  • Marriage or Common-Law
  • Separation
  • Divorce
  • Death of a Spouse

When you begin or end a marriage or common law relationship, you must notify UA Local 67 Benefits to request a new Welfare Plan Enrollment/Change Form and/or a new Pension Plan Enrollment/Change Form. Dependent Changes

  • Birth or Adoption of a Child
  • Children Aged 21 to 24 Attending School
  • Disabled Dependent Children
  • Death of a Child

When your family grows or changes, notify UA Local 67 Benefits to request a new Welfare Enrollment/Change Form and/or a new Pension Plan Enrollment/Change Form. Beneficiary Changes

Member Illness, Injury or Death

Illness & Injury

Death

Upon your death, your spouse, dependents, and named beneficiaries should contact UA Local 67 Benefits as quickly as possible to ensure continued Health & Dental benefits coverage, as well as arranging for life insurance and death benefit or survivor’s pension payments. Learn more about death.

Attending Trade School

If you stop work to attend trade school, you may qualify to receive Supplementary Unemployment Benefits (SUB) weekly payments while you are in school. Learn more attending trade school.

Retirement

Leaving the Plan or Local

If you leave UA Local 67 before you retire, your Pension Plan benefit doesn’t disappear. You have flexible options to help you make the most of what you’ve earned.

Your membership in the UA Local 67 Benefits Welfare Plan ends automatically on the first of the month after the earliest of these events:

  • Your membership in UA Local 67 ends; or
  • You no longer have enough money in your Benefits Account to pay the cost of one month of coverage and you do not make a direct payment to UA Local 67 Benefits.

Important! Make sure you understand the implications for your Pension and Health & Welfare before ending your UA Local 67 (union) membership.
