Electronic Remittance Instructions
Complete your Collective Agreement/Wage Schedule where indicated. The Employer Remittance Form does the rest.
Rates, taxes, and totals are now automated for you. Just choose the Wage Schedule, add the name, SIN, Apprentice level (JM, APR1, APR2, APR3, APR4, APR5) and hours. The form will then calculate the Rate totals, Taxes and Remittance Total for you.
Type your name, title, phone number and email address and check the acknowledgement box. Save your form and send it to us at info@ua67benefits.ca with your Electronic Funds Transfer or print your form and send your cheque to UA Local 67 Benefits, at 102-195 Dartnall Road, Hamilton, ON L8W 3V9.
