$55 per visit ($85 for a first assessment), up to 20 visits per calendar year
Does NOT include the services of:
- Athletic Therapists
- Ergotherapists
- Kinesiologists
- Kinesitherapists
- Muscle Physiologists
- Occupational Therapists
- Orthotherapists
Orthopedic Shoes & Orthotics
What’s Included
Coverage
Orthopedic Shoes, without a brace
$15 per pair, per calendar year
Orthopedic Shoes, as integral part of brace
1 pair, per calendar year
Custom foot orthotics
$300 every 2 calendar years
The following are not considered custom-made and are NOT covered:
- Stock items
- Off-the-shelf orthotics, and prefabricated devices such as cushioned heel cups or insoles
- Shoes purchased for comfort only (e.g. comfort/walking/ running shoes)
- Shoes purchased for sports or recreational activities
- Orthotics made from walking over a pressure pad or plate, making a footprint on an inkpad, or using your shoe size to provide a prefabricated sole or insert
Requirements for Orthopedic Shoes and Orthotics Claims
Given claims for orthopedic shoes and orthotics can be complex, we strongly recommend contacting GreenShield before making a purchase, to ensure you understand the requirements and limitations, specific to your medical needs.
To be reimbursed for orthopedic shoes or orthotics, your claim submission must meet all of GreenShield’s requirements, including:
- Medical diagnosis and prescription from an authorized provider for the making and fitting of prescribed orthotics/orthopedic shoes, from this list of practitioners:
- Podiatrist,
- Chiropodist,
- Pedorthist,
- Orthotist, or
- Chiropractor.
- An itemized receipt showing the date you picked up the shoes/orthotics, and that payment was made in full
- For shoes: the make and model must be included on the itemized receipt
- For orthotics:
- A copy of the biomechanical examination or gait analysis, and identified (3D) casting technique
- A detailed lab invoice or proof of manufacturing (not the manufacturing process document), showing:
- Patient’s name
- Date of invoice or shipping date
- Materials used to construct the orthotic (for onsite labs)
- Invoice number
- From (lab name) and To (clinic name/dispensing provider name)
