Your SUB Plan for Sickness
If you’re facing illness or injury, the Supplemental Unemployment Benefit (SUB) Plan is here to help. Your SUB Plan provides you with $240 per week, up to 10 weeks maximum per calendar year (maximum weeks are shared with other SUB types—unemployment and maternity/parental).
SUB Eligibility
To receive SUB payments, you must:
- Be an active member,
- Have 1,800 SUB hours in the previous 24 months for the first claim, or 600 SUB hours in the previous 12 months for subsequent claims, and
- Be in receipt of EI Sickness benefits.
You are not eligible for the SUB Plan for sickness if you:
- Are unable to work due to a work-related illness or injury—you would apply to WSIB instead.
- Are in receipt of, or previously received, a UA-sponsored pension plan (i.e., after you retire).
How to Apply
To successfully submit your SUB claim, you will need to have applied for Employment Insurance (EI) and gather specific supporting documentation:
- EI Benefit Statement
- Name
- Union number
- Medical Documentation confirming your illness and inability to work
First apply for EI benefits as soon as possible after you stop working.
After you’ve applied for EI:
- Log into your My Service Canada Account
- Navigate to the My Payments section
- Take a screenshot of this page, ensuring that the following information is clearly visible: Your full name, Type of benefit and Net amount paid.
Send this EI Benefit Statement, along with your name and union number to UA Local 67 Benefits using one of the following methods:
- Email: to info@ua67benefits.ca. Make sure to request a confirmation of receipt.
- Canada Post: Mail your required supporting documents.
- In-Person: Drop off your documents at UA Local 67 Benefits.
UA Local 67 Benefits must receive your EI Benefit Statement within 90 days following the date your EI report was processed. If you provide all of the correct supporting documentation and qualify for SUB, you are eligible to receive a SUB payment for the week of the EI waiting period.
Continuing Your SUB Claim
Once your SUB claim is approved, you’ll need to send your EI Benefit Statement details to UA Local 67 Benefits for each week of your EI claim.
- Log into your My Service Canada Account
- Go to My Payment Detail
- Click on the underlined dates for each week of EI under the Report Covering Period section
- Take a screenshot for each week, ensuring that the following information is clearly visible: Your full name, Type of benefit and Net amount paid. Remember to do this for every week, even if the Net Amount Paid is $0.
Submit this information to UA Local 67 Benefits using one of the methods above. Don’t forget to include your name and union number in your submission.
You can submit your supporting documents after each week you receive EI benefits or several weeks at once. However, please note that if you wait to send in multiple weeks together, earlier weeks will not be paid if they are older than 90Â days following the date your EI report was processed.

Additional Support While Off Sick
You have other types of support available to you while on leave:
- Your UA Local 67 Benefits Welfare Plan – While on leave, you are still eligible to access your Welfare Plan benefits.
- Member Assistance Program (MAP) – The UA Canada National Wellness Program offers all active members access to the national MAP, an employee and family assistance program (EFAP) that provides you and your family with immediate and confidential support to help resolve work, health, and life challenges. - Short-term Disability (STD) – Contact the UA Local 67 Benefits team before you have exhausted the 26 weeks of EI Sickness Benefits to see if you qualify for STD.
- UA Local 67 Benevolent Fund Benefits – The UA Local 67 Benevolent Fund offers eligible members off sick $750 per week, up to 3 weeks, for a total of $2,250 from the Benevolent Fund. The benefit covers one occurrence every 12 months. To qualify, the member must not be in receipt of EI, WSIB, disability, or any other monetary benefit.
